Re-Certification Instructions
• Review the Re-Certification Resources.
• Print the Re-Certification Quiz.
• Read the questions and write down your answers.
• Give the completed quiz to a supervisor for review.
• The supervisor will review the Re-Certification Answer Key online.
• Once incorrect answers (if any) are reviewed, and answers are correct, complete the Verification Form and email to:
